Contact for IT Errors

If you are experiencing an error when you try and login to the report, please review the options further below and act according to the option that best suits your situation. If you have to register your issue either through an online web form or via email to IT support staff, please include ALL 9 questions below in your email/web form to assist support staff with your query.

  1. What is your Student/Staff ID number?
  2. What is your contact phone number should we need to call you to discuss the issue?
  3. What is your email address?
  4. Which section/area of http://hdrforms.curtin.edu.au/ have you tried to access?
  5. Which web browser are you using to access HDR Forms website
    1. Web browser name (e.g. Internet Explorer, Firefox)
    2. Web browser version (e.g. IE6, Firefox 2.0.0.13)

  6. Where did you access the website from?
    1. If on-campus, which building?
    2. If off-campus, were you using dial-up or broadband? Please state your connection speed and the name of your Internet Service Provider.
  7. As a student, if you were having problems with document upload/s please state the size of the document and/or attach it to the email for our reference.
  8. If possible, please provide the text of the error message you received and/or a description of the erratic web site behaviour you experienced. If you can provide a screen print of the page with the error this would also be helpful. When on the page with the error simply press the "Print Screen" button on your keyboard (usually at the top right) then go into the Word document and press "Control+V" to paste the image into the document. Please provide as much information as you can.

Login Errors


I am unable to login and get a "Authentication failed:[LDAP: error code 49 - NDS error: failed authentication (-669)]" error message

Students
Are you able to login to OASIS, at http://oasis.curtin.edu.au/, successfully?

If not,

If you have reviewed the questions above but still cannot login to OASIS, please also try to login as a student to the eVALUate survey via http://evaluate.curtin.edu.au/ to find out if this error also occurs with this similar system, or if it is only occurring with the report.
Can you login to eVALUate?

When you have logged the call with OASIS Central they will contact you and advise that they will reset your user details and/or reset your OASIS password. Once either of these actions is done you should then be able to then login to the report.

If OASIS Central reset your user details and/or reset your OASIS password but you are still unable to login to the report, please contact your local IT Helpdesk for assistance and advise them of what action has been taken so far by OASIS Central. Please include ALL 9 questions at the top of this page, along with your answers, in your email to your local IT Helpdesk to assist support staff with your query.

I am unable to login and get a "User [ID NUMBER] is not a valid HDR Forms user" error message

Students

Please contact the relevant Graduate Studies Officer (GRS.CurrentStudents@curtin.edu.au) so they may check your current enrolment details on Student One, the student records system.

If any changes need to be made these changes will flow through to the report overnight so you should be able to login to the system on the following day.

Staff

Please contact the relevant Graduate Studies Officer (GRS.CurrentStudents@curtin.edu.au) so they may check that the Thesis Committee listings are current for your student/s on Student One, the student records system.

I am unable to login and get a "An error has occured while trying to execute sort :[LDAP: error code 53 - NDS error: log account expired (-220)]. One or more of the required attributes may be missing or incorrect or you do not have permissions to execute this operation on the server" error message

Students and Staff
This error message relates to your IT account details and indicates that your Novell account has been disabled in error or has expired. Please contact your local IT Helpdesk for assistance, and please include ALL 9 questions at the top of this page, along with your answers, in your email to assist support staff with your query. I have received a different error message

Students and Staff

If you have received a different error message, please send an email including ALL 9 questions at the top of this page, along with your answers, to the relevant Graduate Studies Officer (GRS.CurrentStudents@curtin.edu.au). The FGSO will then direct your email to the relevant IT area.
